OUR MISSION

Our mission is to help keep patients in the comfort of their homes while improving quality of life through optimal nutrition. Research shows that reducing malnutrition and sarcopenia not only reduces hospital readmissions but also reduces morbidity and mortality. We use evidence-based nutrition to guide our practice, avoiding diagnosis-driven restrictive diets, and instead prioritizing the physical, emotional and social aspects of nutrition for each individual.

OUR PURPOSE

Our top priority is to partner with home health and hospice agencies in the joint effort to keep patients safely in their homes. We help reduce nutrition risk factors that lead to hospital readmissions by providing comprehensive nutrition assessments that focus on individualized nutrition interventions, allowing for a wide range of enjoyable foods.

OUR PHILOSOPHY

Nutrition and hydration play a pivotal role in reducing hospital readmissions and keeping patients comfortably at home. Prioritizing adequate nutrition over diet restrictions can help reduce falls, dehydration, malnutrition, UTIs, infections and other potential life threatening co-morbidities. Literature does not support the routine use of restrictive diets as they can lead to these unfavorable outcomes.

We believe we can help reduce malnutrition and dehydration amongst the elderly by eliminating fears and worry around food restrictions, and promoting the enjoyment of foods and beverages that can adequately nourish and hydrate.

We are committed to providing helpful interventions for patients and families, while incorporating a liberalized diet approach whenever possible.
